Guidelines for forwarding of Final payment cases by DDOs
- Application should be in prescribed form ie. 10-A, 10-B or in case of death in form 10-C dully filled in and signed by the subscriber and attested by DDO/HOD.
- Details of last fund deduction ie. the name of treasury, Voucher Number, Month & amount of deduction
- Details of withdrawal for last 12 months .ie. the name of treasury, Voucher Number, Month & amount of withdrawal
- Details of withdrawal of 90% –if already drawn and if not a certificate by the DDO to the effect that 90% has not been drawn and that the same would not be drawn subsequent to the forwarding of FP application to this office.
- Order of voluntary retirement/dismissal/ resignation.
- Death certificate of the subscriber along with the certificate that the subscriber has expired while in service to issue/authorise DLI payment.
- In case of death of the subscriber.
- If nomination exists the FP authority would be in favour of nominee/nominees as per the share indicated in the nomination.
- If no nomination exists in favour of member/members of the family of the Subscriber, the fund money becomes payable in equal share to the members of his family. The list of family members should be duly certified by the DDO and the Office would process as per the information provided in the application/certification provided by the DDO. Eligibility of the person to be considered as a member of the family would be as per the definition of family, under the GPF Rules.
- If it is a case wherein no nomination exists as well as subscriber not having a family, then the legal heirship is to be established through varsai or other legal mode.
